Driver Permission Form
I, _______________________________ give permission for my child to drive with any parents, employees, or advisors of Our Savior Lutheran Church, as long as there are other people present.
I (circle one) do do not give permission for my child to ride in a car alone with the adults listed below. I will speak with my child to inform them who they are allowed and not allowed to get in a car alone with. I understand that Our Savior Lutheran Church does not condone the practice of minors driving with adults alone, yet will heed to my judgment as a parent. I also clearly understand (I have no questions) that Our Savior Lutheran Church, its staff, both paid and non paid, are held legally responsible for anything that may happen to my child while driving alone with an adult. I further understand that if the adult does not feel comfortable being alone with my child, they reserve the right now to transport my child. Please cross off any person that you do not want your child riding alone with. This form will be kept in the DYCM’s office and will not be released to anyone without prior notification to at least one of the parents named below.
